In order to register your child, you will need to complete the following-
2- Complete registration and pay the annual registration fee of $100
3- Submit a birth certificate for confirmation of ID. We will need to see an original document, which will be viewed, documented, and returned.
4- Submit a copy of the immunization records. These will be kept on file.
5- Complete and submit intake forms
